Abstract

This book trains the next generation of scientists representing different disciplines to leverage the data generated during routine patient care. It formulates a more complete lexicon of evidence-based recommendations and support shared, ethical decision making by doctors with their patients. Diagnostic and therapeutic technologies continue to evolve rapidly, and both individual practitioners and clinical teams face increasingly complex ethical decisions. Unfortunately, the current state of medical knowledge does not provide the guidance to make the majority of clinical decisions on the basis of evidence. The present research infrastructure is inefficient and frequently produces unreliable results that cannot be replicated. Even randomized controlled trials (RCTs), the traditional gold standards of the research reliability hierarchy, are not without limitations. They can be costly, labor intensive, and slow, and can return results that are seldom generalizable to every patient population. Furthermore, many pertinent but unresolved clinical and medical systems issues do not seem to have attracted the interest of the research enterprise, which has come to focus instead on cellular and molecular investigations and single-agent (e.g., a drug or device) effects. For clinicians, the end result is a bit of a “data desert” when it comes to making decisions. The new research infrastructure proposed in this book will help the medical profession to make ethically sound and well informed decisions for their patients.

Abstract

"Health IT is a major field of investment in support of healthcare delivery, but patients and professionals tend to have systems imposed upon them by organizational policy or as a result of even higher policy decision. And, while many health IT systems are efficient and welcomed by their users, and are essential to modern healthcare, this is not the case for all. Unfortunately, some systems cause user frustration and result in inefficiency in use, and a few are known to have inconvenienced patients or even caused harm, including the occasional death. This book seeks to answer the need for better understanding of the importance of robust evidence to support health IT and to optimize investment in it; to give insight into health IT evidence and evaluation as its primary source; and to promote health informatics as an underpinning science demonstrating the same ethical rigour and proof of net benefit as is expected of other applied health technologies. The book is divided into three parts: the context and importance of evidence-based health informatics; methodological considerations of health IT evaluation as the source of evidence; and ensuring the relevance and application of evidence. A number of cross cutting themes emerge in each of these sections. This book seeks to inform the reader on the wide range of knowledge available, and the appropriateness of its use according to the circumstances. It is aimed at a wide readership and will be of interest to health policymakers, clinicians, health informaticians, the academic health informatics community, members of patient and policy organisations, and members of the vendor industry."

Abstract

"This book addresses the topic of toilet design, but instead of focusing on the typical able bodied user, it takes the various needs and limited abilities of older and/or disabled people as its starting point (human centered design). This follows the principles of ‘Inclusive Design’: designs taking into account the needs of the most challenged, will also have benefits for the healthy . For the most part, this book is a spin-off of an EU-funded research and development project called the ‘Friendly Rest Room for Elderly People’ (FFR), which ran from 2001 to 2005. During that period a consortium of 10 institutions in 7 European countries collaborated on the research, design and development of a friendlier toilet for elderly and disabled users. The contributions from partners in the FRR consortium to this book reflect the results of nearly 5 years of empirical work in different cultures, countries and disciplines. The book has been brought up-to-date with developments in this area of research in recent years. The goal of the FFR project was to carry out the necessary research and design, build and test prototypes for a Friendly Rest Room for older people and persons with a disability, enabling them to gain greater autonomy, independence, self-esteem, dignity, safety, improved self-care and thereby enjoy a better quality of life. The methods and technologies involved in fulfilling this objective included contactless smart card technologies with read-write capabilities, voice activation interface, motion control and sensor systems, mechanical engineering and robotic techniques, mathematical modelling, as well as ergonomic research and designs inspired by philosophy, gerontechnology and medical and social sciences. End-users and secondary users, as well as care takers and rehabilitation professionals, were involved in all stages of the research and problem solving processes of the FRR prototype development. Prototypes were tested taking account of the advice of industrial marketing companies and end-user organisations to improve the independence, dignity, safety, self-care and quality of life of older and/or disabled people within the European community. Abstract

La profession de physicien médical est peu connue. Apparue en France il y a une cinquantaine d'années, elle comprend aujourd'hui plus de 600 professionnels qui travaillent au quotidien aux côtés des médecins spécialistes de radiothérapie, radiologie et médecine nucléaire, pour les aider à optimiser l'utilisation des rayonnements ionisants dans la lutte contre le cancer et dans les actes médicaux à visée diagnostique ou thérapeutique. Bénéficiant d'une formation scientifique de haut niveau, les physiciens médicaux sont par définition au carrefour de la médecine et de la physique. Ils ont du mal à trouver leur place dans la classification des métiers et ne sont pas actuellement reconnus en tant que professionnels de santé. De fait, ce n est qu'à la suite des graves accidents de radiothérapie survenus en France il y a quelques années, que les pouvoirs publics ont pris la mesure de l importance de cette profession et du déficit de postes. Plus récemment, suite à l'augmentation des doses reçues par les patients en radiologie interventionnelle et en scanographie, le besoin accru de compétences en physique médicale dans ces domaines est apparu. Ce guide s'adresse prioritairement à ceux qui exercent ou vont exercer le métier de physicien médical. Il vise à les accompagner dans la découverte des méandres réglementaires et des techniques de management de la qualité et de la sécurité. Il se propose d'être un ouvrage de référence pour les aider à accomplir leurs missions dans le respect des principes de bonne conduite et de bonne pratique professionnelle.

Abstract

Flow of ions through voltage gated channels can be represented theoretically using stochastic differential equations where the gating mechanism is represented by a Markov model. The flow through a channel can be manipulated using various drugs, and the effect of a given drug can be reflected by changing the Markov model. These lecture notes provide an accessible introduction to the mathematical methods needed to deal with these models. They emphasize the use of numerical methods and provide sufficient details for the reader to implement the models and thereby study the effect of various drugs. Examples in the text include stochastic calcium release from internal storage systems in cells, as well as stochastic models of the transmembrane potential. Well known Markov models are studied and a systematic approach to including the effect of mutations is presented. Lastly, the book shows how to derive the optimal properties of a theoretical model of a drug for a given mutation defined in terms of a Markov model.

Abstract

The recent developments in biomedical sensors, wireless communication systems, and information networks are transforming the conventional healthcare systems. The transformed healthcare systems are enabling distributed healthcare services to patients who may not be co-located with the healthcare providers, providing early diagnoses, and reducing the cost in the healthcare section. The developments in medical internet of things (m-IoT) would enable a range of applications, including remote health monitoring through medical-grade wearables to provide homecare for elderlies; virtual doctor-patient interaction to have any time and place access to medical professionals; wireless endoscopic examination; and remotely operated robotic surgery to extend the access to highly skilled surgeons. Wireless body area networks (WBAN) are key enablers of these transformations. These networks connect sensors and actuators to external processing units, which could be placed on the surface of the patient's body or implanted inside the body to connect specific sensors and/or actuators inside, on, and around the body to the data collection points. The success of these networks highly relies on the advent of low-power, low-delay, reliable, and low-cost wireless connectivity solutions. This book covers recent developments in wireless healthcare systems to provide an insight to the technological solutions (e.g. for body area channel propagation models, communication techniques, and energy harvesting/transfer) for wireless body area networks, and emerging applications of medical internet of things and wireless healthcare systems.

Abstract

This book investigates what international placements of healthcare employees in low resource settings add to the UK workforce and the efficacy of its national health system. The authors present empirical data collected from a volunteer deployment project in Uganda focused on reducing maternal and new-born mortality and discuss the learning and experiential outcomes for UK health care professionals acting as long term volunteers in low resource settings. They also develop a model for structured placement that offers optimal learning and experiential outcomes and minimizes risk, while shedding new light on the role that international placements play as part of continuing professional development both in the UK and in other sending countries.

Abstract

This year we celebrate the 10th edition of the DeSForM conference series. In 2005, DeSForM was born in (Eindhoven) the Netherlands with the premise of creating meaning through objects, interactions and people. Over the years, the DeSForM community has explored and designed objects through a multi-sensorial approach always aiming at enriching users' experiences with them. Throughout previous editions, we have seen the development from digital and mechanical objects that had enriched sensorial presence to adaptive and intelligent objects that feel almost analogous to reality given their increased information processing power and sensory resolution. These are thanks to the recent developments in the material sciences, robotics, information and sensor technology, and improved production techniques. Consider, for example, developments in wearables and embedded or computational materials. Consequently, the arena that belongs to design researchers and practitioners has gotten more sophisticated by being more technical, but also raises new questions regarding the effect and the impact of the new technologically rich designs. In 2017, DeSForM is returning to its place of birth opening up to a broader audience with deeper insights to debate about the future of dynamic 'form' giving and its effects on people and their environment.
