Abstract

Over the past decades, surgical techniques have greatly progressed to improve and correct appearance. They are artistic procedures to give the highly demanded proportions. The growing public interest in aesthetic and plastic surgery interventions such as body contouring and sculpting requires clear description and differentiation of these highly sophisticated techniques, their results and combinations, as well as scientific information about the different instrumentation, devices and materials used. Such clarification will be hugely beneficial both for patients and doctors, having in mind the increasing number of such interventions and progressing interest for a better and healthy living including improved appearance in society. The book adds some scientific news to the understanding of body contouring treatments. Currently, the request for atraumatic, short downtime procedures predominates and demonstrates the importance of hi-tech and safe liposculpture and fat transfer, although they cannot totally replace surgical excision methods.

Abstract

Immune response and metabolic regulation are highly integrated and this interface maintains a central homeostatic system, dysfunction of which can cause obesity-associated metabolic disorder such as type 2 diabetes, fatty liver disease and cardiovascular disease. Insulin resistance is an underlying basis for the pathogenesis of these metabolic diseases. Overnutrition or obesity activates the innate immune system with subsequent recruitment of immune cells such as macrophages and T cells, which contributes to the development of insulin resistance. In particular, a significant advance in our understanding of obesity-associated inflammation and insulin resistance has been recognition of the critical role of adipose tissue macrophages (ATMs). ATMs are a prominent source of proinflammatory cytokines, such as TNF-a and IL-6, that can block insulin action in adipose tissue, skeletal muscle, and liver autocrine/paracrine signaling and cause systemic insulin resistance via endocrine signaling, providing a potential link between inflammation and insulin resistance. All articles in this topic highlight the interconnection between obesity, inflammation, and insulin resistance in all its diversity to the mechanisms of obesity-induced inflammation and role of immune system in the pathogenesis of insulin resistance and diabetes.

Abstract

Eating Disorders have traditionally been considered apart from public health concerns about increasing obesity. It is evident that these problems are, however, related in important ways. Comorbid obesity and eating disorder is increasing at a faster rate than either obesity or eating disorders alone and one in five people with obesity also presents with an Eating Disorder, commonly but not limited to Binge Eating Disorder. New disorders have emerged such as normal weight or Atypical Anorexia Nervosa. However research and practice too often occurs in parallel with a failure to understand the weight disorder spectrum and consequences of co-morbidity that then contributes to poorer outcomes for people living with a larger size and an Eating Disorder. Urgently needed are trials that will inform more effective assessment, treatment and care where body size and eating disorder symptoms are both key to the research question.

Abstract

The prevalence of diabetes is on the increase in the UK and worldwide, partly due to changes in lifestyle which predispose individuals to overweight and obesity. It is estimated that about 90% of the currently diagnosed adults have type 2 diabetes, and based on the World Health Organisation (WHO) report, about 422 million adults were living with diabetes in 2014 compared with 108 million in 1980; this condition caused about 1.5 million deaths in 2012. In the United States of America, it is estimated that about 30.3 million adults are living with diabetes, with a further 1.5 million new diabetes cases diagnosed every year, representing an increasing prevalence of this condition. Diabetes represents a major public health challenge, despite advances in technology and the pharmaceutical industry. These problems may be in the form of acute or long-term complications. Therefore, in order to attenuate the problems of diabetes, management strategies usually include lifestyle changes such as increased physical activity and dietary interventions. Studies which evaluate the role of nutrition in the management of type 2 diabetes often involve human and animal models as these approaches enable us to have a broader and more in-depth understanding of the condition. In some cases, diabetes may co-exist with other conditions, such as stroke, and these may present unique challenges with regard to nutritional interventions. This Special Issue aims to evaluate the risk factors associated with type 2 diabetes and the role of the diet in the management of people with this condition. This evidence is drawn from both human and animal studies.
