Abstract

Homecare ist ein junger Bereich, der die stationäre Versorgung mit der ambulanten Nachsorge verbindet und sämtliche weitere Versorger koordiniert. In der Praxis scheitert diese Idee derzeit noch häufig an fehlenden oder unpräzisen rechtlichen Rahmenbedingungen sowie Akzeptanzbarrieren. Dieses Buch bietet einen umfassenden Überblick der aktuellen Versorgungssituation im Homecare-Bereich in Deutschland. Es diskutiert den Homecare-Markt und seine Potentiale, die rechtlichen Rahmenbedingungen und gibt einen Überblick über nationale und internationale Studien zur Effektivität von Entlassmanagement und sektorenübergreifender Versorgung. Ein Praxisteil präsentiert Ergebnisse einer umfassenden empirischen Studie, die die Perspektiven von niedergelassenen Ärzten, Kliniken und stationärer sowie ambulanter Pflege zum Thema Homecare untersucht. Die Ergebnisse der Studie geben Aufschluss über die praktische Umsetzung der Homecare-Idee im Versorgungsalltag sowie die Potentiale und Risiken.

Abstract

Dieses Open-Access-Buch bündelt technische, wirtschaftliche, medizinische und ethische Reflexionen über Pflegeroboter. Pflegeroboter, im Moment noch mehrheitlich Prototypen, unterstützen oder ersetzen menschliche Pflegekräfte bzw. Betreuer. Sie bringen Kranken und Alten die benötigten Medikamente und Nahrungsmittel, helfen beim Hinlegen und Aufrichten oder alarmieren den Notdienst. Vorteile von Pflegerobotern sind durchgehende Verwendbarkeit und gleichbleibende Qualität der Dienstleistung. Nachteile sind Kostenintensität (bei möglicher Amortisation) und Komplexität der Anforderungen. Unter der wissenschaftlichen Leitung von Prof. Dr. Oliver Bendel trafen sich im September 2017 Vertreter verschiedener wissenschaftlicher Disziplinen im Rahmen eines Ladenburger Diskurses der Daimler und Benz Stiftung, um über den aktuellen und künftigen Einsatz von Pflegerobotern zu sprechen und Forschungspotenziale zu identifizieren. Die Autoren gehen in ihren Beiträgen auch Fragen aus Wirtschafts-, Medizin- und Informationsethik nach: Wer trägt die Verantwortung bei einer fehlerhaften Betreuung und Versorgung durch die Maschine? Inwieweit kann diese die persönliche und informationelle Autonomie des Patienten unterstützen oder gefährden? Ist der Roboter eine Entlastung oder ein Konkurrent für Pflegekräfte? Antworten müssen von Wissenschaft und Gesellschaft gefunden werden.

Abstract

Resilience has become an important topic on the safety research agenda and in organizational practice. Most empirical work on resilience has been descriptive, identifying characteristics of work and organizing activity which allow organizations to cope with unexpected situations. Fewer studies have developed testable models and theories that can be used to support interventions aiming to increase resilience and improve safety. In addition, the absent integration of different system levels from individuals, teams, organizations, regulatory bodies, and policy level in theory and practice imply that mechanisms through which resilience is linked across complex systems are not yet well understood. Scientific efforts have been made to develop constructs and models that present relationships; however, these cannot be characterized as sufficient for theory building. There is a need for taking a broader look at resilience practices as a foundation for developing a theoretical framework that can help improve safety in complex systems. This book does not advocate for one definition or one field of research when talking about resilience; it does not assume that the use of resilience concepts is necessarily positive for safety. We encourage a broad approach, seeking inspiration across different scientific and practical domains for the purpose of further developing resilience at a theoretical and an operational level of relevance for different high-risk industries. The aim of the book is twofold: 1. To explore different approaches for operationalization of resilience across scientific disciplines and system levels. 2. To create a theoretical foundation for a resilience framework across scientific disciplines and system levels. By presenting chapters from leading international authors representing different research disciplines and practical fields we develop suggestions and inspiration for the research community and practitioners in high-risk industries. Abstract

This open access book is a collection of 12 case studies capturing decades of experience improving health care and outcomes in low- and middle-income countries. Each case study is written by healthcare managers and providers who have implemented health improvement projects using quality improvement methodology, with analysis from global health experts on the practical application of improvement methods. The book shows how frontline providers in health and social services can identify gaps in care, propose changes to address those gaps, and test the effectiveness of their changes in order to improve health processes and outcomes.
