Abstract

Heart valves are the basic functional structure of the heart to obtain, sustain, and maintain blood circulations in the body without energy loss, allowing blood regurgitation or obstructing blood flow. There are two basic designs of heart valves: actively protected via subvalvular apparatus (atrioventricular valves) and passively strengthened (ventriculoarterial valves). The embryologic and morphologic development of heart valves is the key stage to continue normofunctional structure and physiological behavior. Fetal anomalies, embryologic defects, or non-development pathologies may damage the structure of heart valves and cause functional deterioration at different grades. In this book, we have focused on specific pathologies, which require special knowledge and treatment modalities.

Abstract

The endothelium enables communication between blood and tissues and is actively involved in cardiovascular homeostasis. Endothelial dysfunction has been recognized as an early step in the development of cardiovascular diseases: respectively, endothelium represents a potential therapeutic niche with multiple targets. The purpose of the book is to point out some recent findings of endothelial physiology and pathophysiology emphasizing various aspects of endothelial dysfunction connected to the body's internal and external environment. Abstract

Congenital Heart Disease is a general term for a range of birth defects that affect the normal workings of the heart. It is one of the most common types of birth defect occurring in 1% of live births and affects up to 9 in every 1,000 babies born in the United Kingdom. It was estimated that 34.3 million people had a congenital heart abnormality in 2013. Abnormalities can arise in 3-5% of off spring in cases of de novo family history. Congenital heart disease was responsible for 223,000 deaths in 2010. This short volume details the common birth defects affecting the structure and functioning of the heart concentrating on the genetic basis and epidemiology, as well as risk factors, diagnostic modalities and treatment options.

Abstract

Atherosclerotic cardiovascular disease is still the most common cause of death among adults. Its prevalence is increasing in developing countries and despite all advances in both diagnostic tools and treatment modalities, it is still very common in the developed world. Obesity, diabetes mellitus, hypercholesterolemia and overuse of dietary salt play a pivotal role in increased cardiovascular morbidity and mortality worldwide. Current clinical efforts are mainly focused on the diagnosis and treatment of myocardial infarction. In this book, we provide epidemiological data on myocardial infarction and atherosclerotic cardiovascular disease, current diagnostic biochemical tests and management strategies. A specific patient group, children, experiencing myocardial infarction are also addressed. Current advances in the management of myocardial infarction have decreased the morbidity and mortality from atherosclerotic cardiovascular disease and especially myocardial infarction; however, more can be achieved by the prevention of atherosclerotic processes via focusing on the early stages of the disease.

Abstract

Extracorporeal membrane oxygenation (ECMO) has evolved into an exciting and valuable tool to assist in the management of patients experiencing cardiogenic shock, severe acute respiratory failure, or often a combination of both. While outcomes remain less than ideal, they continue to improve with team experience, better patient selection, and a growing understanding of the nuances of managing patients who require mechanical circulatory support. Patients requiring ECMO are often extremely sick and have complex problems - initiating therapy before the development of end-organ damage is critical. Without doubt, teamwork, guidelines, and protocols are cornerstone concepts for clinical and program success - all topics that are emphasized in this text. The goals of this text are to further outline topics that help address some of the key challenges providers face when considering and applying extracorporeal support therapies to the evolving spectrum of acutely ill patients.
