Abstract

In this chapter, I explore the regulation of alternative and traditional medicine, in&#xD;order to reflect on how particular temporalities shape, and are shaped by, the&#xD;interface between law and medicine. This chapter makes two key points: first, it&#xD;argues that both biomedicine and law have relied on a particular sense of&#xD;‘modernity’ as a linear temporal process; in turn, this has been key in developing&#xD;both crude, and more subtle, social patterns of power, dominance, and exclusion&#xD;that continue to impact on contemporary societies. Second, it argues that as law&#xD;increasingly engages in the regulation of other types of medicine, it continues to&#xD;emulate biomedical models and assumptions as to what ‘modern medicine’&#xD;should look like, including its temporal features. This chapter is written as I am&#xD;starting a large investigation of the multiple ways in which traditional and&#xD;alternative medicines apprehend and are apprehended by law in several states in&#xD;Europe and Africa.2 Although the project has several aims, my interest in the field&#xD;came, in part, from the ambivalent and complex ways in which the idea of&#xD;‘modernity’ seemed to be shaping the field and, in turn, how law and medicine&#xD;as institutions were involved in this ambivalence. It is useful, to ‘set the scene’ of&#xD;this chapter, to return to this briefly.

Abstract

The field of Syriac medicine is perhaps one of the least investigated and explored&#xD;domains within Syriac intellectual culture. Yet owing to its decisive role during the&#xD;late antique period for the transfer of Greek medical knowledge to the Islamic world,&#xD;it should occupy a very special position, and the results of its study are appealing to&#xD;both Classicists and historians of Greek and Islamic medicine. The study of Syriac&#xD;medicine deals predominantly with medical literature, but also with theory and practice&#xD;as they evolved over centuries within changing social and historical contexts&#xD;(the surveys available differ in perspective and scope: Gignoux 2001a ; Habbi 2001 ;&#xD;Muraviev 2014 ; Pormann and Savage-Smith 2007 : 17–21; Strohmaier 1994 ; on the&#xD;Syriac scholarly milieu more broadly: Debié 2014 ).

Abstract

A vast amount has been written about NIV, including books and guidelines hence we thought to produce a book called ""Noninvasive Ventilation in Medicine - Recent Updates"" to cover the untouched components of such this machine. In this book, we tried to include advances in the NIV and the how NIV could be used in synchrony with the mechanical ventilator including a weaning stage. The clinical scope of NIV is changing day-to-day and its rapidly emerging and constantly changing field includes many more indications of utilization of NIV. The current book contains a rich extract from the masters in the NIV field who have vast experience of NIV in areas other than conventional indications and would like to share their experience with all of the readers. Various challenges in NIV patient care include noncompliance, confused, hypercapnic patient or small children coping with a mask, avoiding interface leaks, and balancing ventilatory needs with patient tolerance.

Abstract

This book is open access under a CC BY 4.0 license. It constitutes a unique source of knowledge and guidance for all healthcare workers who care for patients with sepsis and septic shock in resource-limited settings. More than eighty percent of the worldwide deaths related to sepsis occur in resource-limited settings in low and middle-income countries. Current international sepsis guidelines cannot be implemented without adaptations towards these settings, mainly because of the difference in local resources and a different spectrum of infectious diseases causing sepsis. This prompted members of the Global Intensive Care working group of the European Society of Intensive Care Medicine (ESICM) and the Mahidol-Oxford Tropical Medicine Research Unit (MORU, Bangkok, Thailand) - among which the Editors – to develop with an international group of experts a comprehensive set of recommendations for the management of sepsis in resource-limited settings. Recommendations are based on both current scientific evidence and clinical experience of clinicians working in resource-limited settings. The book includes an overview chapter outlining the current challenges and future directions of sepsis management as well as general recommendations on the structure and organization of intensive care services in resource-limited settings. Specific recommendations on the recognition and management of patients with sepsis and septic shock in these settings are grouped into seven chapters. The book provides evidence-based practical guidance for doctors in low and middle income countries treating patients with sepsis, and highlights areas for further research and discussion.

Abstract

Atherosclerotic cardiovascular disease is still the most common cause of death among adults. Its prevalence is increasing in developing countries and despite all advances in both diagnostic tools and treatment modalities, it is still very common in the developed world. Obesity, diabetes mellitus, hypercholesterolemia and overuse of dietary salt play a pivotal role in increased cardiovascular morbidity and mortality worldwide. Current clinical efforts are mainly focused on the diagnosis and treatment of myocardial infarction. In this book, we provide epidemiological data on myocardial infarction and atherosclerotic cardiovascular disease, current diagnostic biochemical tests and management strategies. A specific patient group, children, experiencing myocardial infarction are also addressed. Current advances in the management of myocardial infarction have decreased the morbidity and mortality from atherosclerotic cardiovascular disease and especially myocardial infarction; however, more can be achieved by the prevention of atherosclerotic processes via focusing on the early stages of the disease.

Abstract

Extracorporeal membrane oxygenation (ECMO) has evolved into an exciting and valuable tool to assist in the management of patients experiencing cardiogenic shock, severe acute respiratory failure, or often a combination of both. While outcomes remain less than ideal, they continue to improve with team experience, better patient selection, and a growing understanding of the nuances of managing patients who require mechanical circulatory support. Patients requiring ECMO are often extremely sick and have complex problems - initiating therapy before the development of end-organ damage is critical. Without doubt, teamwork, guidelines, and protocols are cornerstone concepts for clinical and program success - all topics that are emphasized in this text. The goals of this text are to further outline topics that help address some of the key challenges providers face when considering and applying extracorporeal support therapies to the evolving spectrum of acutely ill patients.
