Abstract

Occupational therapy is a health care specialty with a deep focus on client-centered and holistic rehabilitation to improve the individual's occupational performance, quality of life and well-being through participation in meaningful and purposeful activities. This new book presents the importance of the therapeutic and creative use of activity in different populations, which is one of the core components of occupational therapy. Rehabilitation, rehabilitation delivery and outcomes are affected by recent changes in the meaning of health and social values. This resulted in an increasing necessity for therapeutic therapy, as well as creative use of activity in occupational therapy. This book focuses on recent advances in occupational therapy and reviews current practical guidelines. Abstract

Obesity and eating disorders remain major public health concerns worldwide that often co-exist. While we are far from having definitive solutions, scientific insights into the prevention and management of obesity and eating disorders have developed significantly in recent years. This research has been stimulated by the continuous challenge of reducing the global burden of obesity and its commonly associated health complications, as well as the life-threatening characteristics of eating disorders. Looking towards better solutions for these pressing public health concerns, this book addresses recent scientific findings regarding the prevention and management of obesity and eating disorders. It reports on cognition and mental health in people with obesity, as well as on food addiction, co-morbidities of eating disorders, cultural aspects of eating, intermittent dietary energy restriction as a treatment for overweight or obesity, factors influencing the adherence to dietary treatments for overweight and obesity, philosophies underpinning dietary practices, treatment of eating disorders in men, cognitive remediation therapy for anorexia nervosa, and online or virtual interventions for weight management or bulimia nervosa. Abstract

Please describe the book in straightforward and consumer-friendly terms.[This Special Issue book, “Natural Products for Cancer Prevention and Therapy”, is based on recent advances in natural products for cancer prevention and therapy. For this purpose, the authors of this book have been organizing a biennial international conference series. The first meeting (First International Conference on Natural Products for Cancer Prevention and Therapy) was held in Istanbul between 31 August and 2 September 2015, with the support and contribution of many valuable researchers in this field. The abstracts of the first conference were published in the Anticancer Drugs journal as a supplement. The second meeting, namely The Second International Conference on Natural Products for Cancer Prevention and Therapy, was held at Erciyes University, Kayseri, Turkey, between 8 and 11 November 2017. The abstracts of all of the presentations from the second meeting were published in the Special Issue of Proceedings by the MDPI publishing group. Furthermore, the conference report and the selected full-length papers based on the presentations at the meeting, as well as other papers based on natural products for cancer prevention and therapy, were published as a Special Issue of the Nutrients journal from MDPI. This Special Issue has contributions from various participants of the aforementioned conference, as well as other cancer and natural product researchers. Abstract

There has been an observed decrease in the global mortality from cancer, mostly atributable to improved, particularly early, detection and prevention. For many carcinomas and leukaemias in adults, once the disease has reached a certain stage there are no therapies that are able to erradicate the cancer cells and cure patients. There has been progress in the treatment of acute myeloid leukaemia (AML) and remissions are achievable; however, the presence of chemoresistent blast cells leads to most patients relapsing, and relapse is difficult to treat and thus patients die due to their disease. Targeting these resistent cells and the leukaemia stem cells, which sustain the leukaemia, is crucial for an effective therapy for AML. Moreover, an increasing number of diverse mutations have been described in AML cells that disrupt the ability of these cells to undergo differentiation. The use of pro-differentiating agents to drive the blast cells to mature, and subsequently undergo apoptosis, provides another approach to therapy. Differentiation therapy, using all-trans retinoic acid (ATRA), an inducer of granulocyte differentiation, has been highly successful in the case of acute promyeloicytic leukaemia, a sub-type of AML, turning this disease into a curable malignancy.

Abstract

MicroRNAs (miRNAs) constitute a large family of small, approximately 20–22-nucleotide, non-coding RNAs that are involved in gene regulation, mainly at the post-transcriptional level. Multiple lines of evidence have indicated that miRNAs play important roles in the maintenance of biological homeostasis and that aberrant expression levels of miRNAs are involved in the onset of many diseases, including cancer. In various types of cancer, miRNAs play important roles in tumor initiation and development. Recently, miRNAs have been demonstrated to also be secreted via small endosome-derived vesicles called exosomes—which are derived from multiple cell types—including immunocytes and cancer cells. Exosomal miRNAs exert important functions in cell-to-cell communication and have been investigated as prognostic and diagnostic biomarkers.

Abstract

The immune system has the double role of maintaining tissue integrity and homeostasis and of protecting the organism from possible dangers, from invading pathogens to environmentally-borne dangerous chemicals. New chemicals recognisable by the immune system are engineered nanomaterials/ nanoparticles, new agents in our environment that are becoming common due to their presence in many products, from constructions and building material (e.g., solar cells, pigments and paints, tiles and masonry materials) to daily products (e.g., food packaging, cosmetics, and cigarettes). Human beings can be accidentally exposed to engineered nanomaterials when these are released from products containing them or during production in workplaces. Furthermore, intentional exposure occurs in medicine, as engineered nanoparticles are used as tools for improving delivery of drugs and vaccines, vaccine adjuvants and contrast agents in therapeutic, preventive and diagnostic strategies. Nanoparticles that come in contact with the immune system after unintentional exposure need to be eliminated from the organism as they represent a potential threat. In this case, however, due to their peculiar characteristics of size, shape, surface charge and persistence, nanoparticles may elicit undesirable reactions and have detrimental effects on the immune system, such as cytotoxicity, inflammation, anaphylaxis, immunosuppression. Conversely, nanomedicines need to escape immune recognition/elimination and must persist in the organism long enough for reaching their target and exerting their beneficial effects. Immune cells and molecules at the body surface (airway and digestive mucosae, skin) are the first that come in contact with nanomaterials upon accidental exposure, while immune effectors in blood are those that more easily come in contact with nanomedical products. Thus, evaluating the interaction of the immune system with nanoparticles/nanomaterials is a topic of key importance both in nanotoxicology and in nanomedicine. Immuno-nanosafety studies consider both accidental exposure to nanoparticles, which may occur by skin contact, ingestion or inhalation (at doses and with a frequency that are not known), and medical exposure, which takes place with a defined administration schedule (route, dose, frequency). Many studies focus on the interaction between the immune system and nanoparticles that, for medical purposes, have been specifically modified to stimulate immunity or to avoid immune recognition, as in the case of vaccine carriers/adjuvants or drug delivery systems, respectively. The aims of this Research Topic is to provide an overview of recent strategies: 1.for assessing the immunosafety of engineered nanomaterials/nanoparticles, in particular in terms of activation of inflammatory responses, such as complement activation and allergic reactions, based on the nanomaterial intrinsic characteristics and on the possible carry-over of bioactive contaminants such as LPS. Production of new nanoparticles taking into account their effects on immune responses, in order to avoid undesirable effects on one hand, and to design particles with desirable effects for medical applications on the other hand; 2.for designing more effective nanomedicines by either avoiding or exploiting their interaction with the immune systems, with particular focus on cancer diagnosis and therapy, and vaccination. This collection of articles gives a comprehensive view of the state-of-the-art of the interaction of nanoparticles with the immune system from the two perspectives of safety and medical use, and aims at providing immunologists with the relevant knowledge for designing improved strategies for immunologically safe nanomaterial applications.

Abstract

Macroautophagy, the major lysosomal pathway for recycling intracellular components including whole organelles, has emerged as a key process modulating tumorigenesis, tumor–stroma interactions, and cancer therapy. An impressive number of studies over the past decade have unraveled the plastic role of autophagy during tumor development and dissemination. The discoveries that autophagy may either support or repress neoplastic growth and contextually favor or weaken resistance and impact antitumor immunity have spurred efforts from many laboratories trying to conceptualize the complex role of autophagy in cancer using cellular and preclinical models. This complexity is further accentuated by recent findings highlighting that various autophagy-related genes have roles beyond this catabolic mechanism and interface with oncogenic pathways, other trafficking and degradation mechanisms and the cell death machinery. From a therapeutic perspective, knowledge of how autophagy modulates the tumor microenvironment is crucial to devise autophagy-targeting strategies using smart combination of drugs or anticancer modalities. This eBook contains a collection of reviews by autophagy researchers and provides a background to the state-of-the-art in the field of autophagy in cancer, focusing on various aspects of autophagy regulation ranging from its molecular components to its cell autonomous role, e.g. in cell division and oncogenesis, miRNAs regulation, cross-talk with cell death pathways as well as cell non-autonomous role, e.g. in secretion, interface with tumor stroma and clinical prospects of autophagy-based biomarkers and autophagy modulators in anticancer therapy. Abstract

A critical problem in resource-scarce countries across the globe is the shortage of appropriately trained health care providers. According to the World Health Organization, the current global health workforce shortage of 7.2 million providers is estimated to increase to 12.9 million by 2035. This disproportionately affects resource-scarce countries, denying basic health care to millions and limiting access to life-saving treatments. Due to limited resources in these countries, not enough health professionals receive training, few have the opportunity for continuing education, and the ability to develop or implement educational programs and curricula is constrained. Additionally, many existing providers choose to emigrate in pursuit of professional advancement opportunities, contributing to the overall shortage of qualified health care providers in these environments. Efforts to strengthen health workforce capacity not only increases access, safety and availability of care, but is critical to building resilient health systems capable of caring for the world’s neediest populations. This requires not only cultivating new health care providers, but also providing ongoing professional development to retain and support current providers, advancing the level of practice in accordance with current clinical science, cultivating educators, and enhancing training curricula. It is critical also to contribute to the limited body of research documenting the effectiveness and impact of various models of collaborative education and partnership to improve health worker training and retention. This Research Topic examines strategies for building health workforce capacity through the prism of educational partnerships, offering significant examples of effective models of international collaborative education as well as insight and guidance on the structure and operation of successful global partnerships. Collectively, the 31 articles accepted and included in this eBook represent a diversity of health professions and geographies across academic, non-governmental organizations and other global partnership forms. The published manuscripts highlight various elements of partnerships with several consistent themes emerging: capacity building, local empowerment, mutual trust and respect, long-term commitment, equity, collaboration, and the importance of integrating theory and practice, for a balance of academic and clinical development. The manuscripts provide examples of partnership and educational programs that are in the formative, early stages of implementation and others which have been sustained long term, some for decades.The following eBook is divided into two parts, with each part broken down into sections. Part I of the eBook includes 18 manuscripts that showcase long-term educational programs that strongly exemplify multiple, foundational aspects of international partnerships in education including mutual collaboration and project management, empowerment of host partners to lead and sustain programs, and capacity building. While individual manuscripts included in Part I look broadly at multiple aspects of successful, international partnerships in education, Part II manuscripts focus intently on one-two elements. Part II includes 13 articles that highlight partnership through short- rather than long-term educational initiatives as well as program development and broad academic partnerships.This Research Topic was sponsored by Health Volunteers Overseas – a United States based non-profit that collaborates with over eighty international universities and health institutions to send volunteer health professionals to low-resource countries to provide continuing education, train the trainer courses, professional support, and consultation on academic program and curricula development.

Abstract

In the perioperative setting and in intensive care medicine, early and effective hemodynamic management including fluid therapy and administration of vasoactive drugs to maintain vital organ perfusion and oxygen delivery is mandatory. Understanding the different approaches in the management of critically ill patients during the resuscitation and further management is essential to initiate adequate context- and time-specific interventions. Optimization of hemodynamic variables to achieve a balance between organ oxygen delivery and consumption is a cornerstone. In general, cardiac output (i.e., the blood flow) is considered a major determinant of oxygen supply and thus its monitoring is regarded helpful. However, indicators of oxygen requirements are equally necessary to assess adequacy of oxygen supply. Currently, more and more less or even totally non-invasive monitoring systems have been developed and clinically introduced, but they require validation in particular clinical settings. Cardiac output monitors and surrogates of organ oxygenation only enable to adequately guide management, as patient’s outcome is determined by acquisition and interpretation of accurate measurements, and finally, suitable management decisions.This Research Topic focuses on the currently available techniques, especially the less and non-invasive ones, in the field of hemodynamic monitoring in the perioperative setting and in critically ill patients while summarizing their advantages and limitations.
